Archaic sheet music transcription?

Volume 2 of Percy's Reliques of Ancient English Poetry has a plate with the music to "For the Victory at Agincourt" (c. 15th–17th century?). I, unfortunately, do not know how to read this vintage of music. Could I interest you in transcribing the page? I suggest that the blackpetrucci noteheads are used. The diamond-shape is a semibreve (whole-note), while the square is a breve. The short vertical lines (e.g. at the beginning of first line of the discantus) are rests (see this). If there's anything I can do to assist with this one, let me know.
